According to information from Captain David Pippin of the Ripley County Sheriff's Department, the Lewis vehicle was traveling north on Michigan Road, just south of County Road 800 South, when Lewis lost control of the 1997 Chevrolet Cavalier she was driving. The vehicle slid across the roadway and impacted a tree directly on the drive side door. Lewis was pronounced dead at the scene.

Gary was in a child safety seat in the back seat and was not injured. Both were transported to University Hospital in Cincinnati via Air Care, due to the violent impact.

Officers noted that seat belts and proper child restraints were in place.

Emergency services personnel on the scene included the New Marion Fire Department, Versailles Truck 61 for extrication, Rescue 69, the Indiana State Police and the Ripley County Sheriff's Department.
